Emergency Contact/Alternative Dismissal

I acknowledge that participation in the children's ministry event involves inherent risks, and I, for myself, my minor child and for the child’s other parent(s) and/or guardian(s), hereby release, waive, discharge, and covenant not to sue The Bridge United Methodist Church, and its officers, directors, employees, agents, representatives, volunteers, successors and assigns of and from all liability, loss, claims, demands, possible causes of action, court costs, attorneys’ fees and other expenses arising from any dispute or lawsuit or that may otherwise accrue from any loss, damage or injury to my child’s person or property in any way resulting from or connected with my child’s attendance at a Bridge Kids activity, including, without limitation, the failure of anyone to enforce rules and regulations, failure to make inspections, or the negligence of other persons or of my child.
Consent to Medical Treatment
In the event my child becomes ill or injured, I give my permission for a staff member, volunteer, or representative of The Bridge United Methodist Church to take whatever steps are reasonably necessary to seek or render emergency first aid to my child. I also consent to such emergency medical treatment as may be reasonably necessary to insure the health and welfare of my child. The Bridge shall have no responsibility for administration of routine or daily medications.
I understand that, unless I decline permission below, my child’s attendance at a Bridge Kids ministry program gives The Bridge United Methodist Church permission to use my child’s image and any photos/video recordings which may be taken during the event for promotional purposes in local newspapers and its publications, and for inclusion in its Facebook pages, website, brochures, or other church-related media. I acknowledge that The Bridge may choose not to use any photos/video recordings at the time but still may do so at its own discretion at a later date.
